Please see Service Agreement and Appendices Provided for an explicit schedule of per diem supports and services.
Placement Bed – The baseline of Heartwood’s services are to offer residential services to our clients. This includes a fully furnished private room with an appropriate bed, mattress, and clothing storage with options to personalize each room to the resident’s preferences and interests. The term “placement bed” also includes required services including all meals and snacks, personal care items, seasonal review and purchase of necessary clothing, over the counter medication, transportation, programming supplies, and gifts.
Direct Therapeutic Support Workers – Our direct support team is staffed by therapeutic support workers, who are experts in supporting individuals with developmental disabilities. Support workers provide 24/7 support and supervision to each individual in the home while implementing regular individualized and/or group programming. Our specialized therapeutic support staff are experienced in strategies to best support the success of our clients. We recognize each participant’s unique needs and work to support residents through strength-based programming and a client-centered lens.
Please note that Direct Therapeutic Support Workers may also be referred to as a Direct Support Worker or Support Staff.
Resident Management Team- Each resident at Heartwood receives support from the full management team, including Case Manager, Program Supervisor, and On-Call Crisis Support. Each client is assigned an individual Case Manager to facilitate the overall care and wellbeing of clients, with the goal helping each individual resident thrive. Case Management services include assessing treatment and service needs, developing individual support plans and client-centered goal-planning. Case managers facilitate coordination with external services including medical, psychological and behaviour, social, vocational, and educational supports to ensure each resident achieves their full potential across dimensions. Case Managers work closely with the home’s Program Supervisor. A Program Supervisor provides daily in-home support shared amongst all residents of the home, available for programming guidance and resources, home administration needs, and direct support as needed. Case Managers and Program Supervisors collaborate with residents, family, services agencies and other members of the resident’s support circle on the resident’s care. Lastly, all residents and staff have the 24/7 support from our On-Call Manager, for safety planning and implementation, incidentals and reporting, and additional direct support as needed.
Community Participation – Heartwood encourages residents to be involved in all the local community has to offer to address individual interests and hobbies of our residents. Services include regular involvement in public activities offered in our community (i.e. parades, music events, fairs, sporting events, etc.). Heartwood also encourages involvement in organized sports, lessons and activities. Heartwood has financial programming available, where residents have the opportunity to earn and manage real money. Each resident of Heartwood will receive a free YMCA membership upon request to support local social, recreational and physically active programming. Additionally, Heartwood will support adults in secondary and post-secondary education by providing basic school supplies, and access to some school-based events or programming.
